School Nurse

The COVID Vaccine clinic for students aged 12 and older is from 8am-11am, May 25 at the Telstar Cafeteria. The second vaccine will be available between 8am and 11am on June 2. Students require a consent form (see below).

We are pleased to inform you that we have partnered with the Western Maine Health clinic to provide an opportunity for your student aged 12 and above to receive the Pfizer COVID 19 vaccine at school at no cost to you.  We are asking you to complete the consent forms, if you choose to have your child vaccinated. That will give us an idea of numbers for the clinic.

This vaccine requires 2 doses given 21 days apart. Our first clinic at Telstar will be May 25th and the second one on June 15th. Staff from Western Maine Health will manage the registration process and clinical staff, along with myself, will administer the vaccine, and do the 15 minute required observation period. We will also have Bethel Rescue on site as an extra precaution as we are not a healthcare facility.

Here is a vaccination fact sheet: Pfizer for Caregivers and Recipients

Enclosed are an information sheet and 2 forms which you will need to complete in their entirety and return to my office no later than May 19th:

If you have any questions while completing the forms, please feel free to contact me.
